Went to a professional for help here but was told to use clerical entry level yet he demanded that the dates come first. I am totally lost here.

Are you talking about writing a resume'?
I would put it like this:

January 2005 - April 2006

Sales Associate - The Gap

New York City, NY

- folded clothes

- used cash register

- blah blah blah

August 2004 - December 2004

Cashier - McDonalds

Sleepytown, NB

- used cash register

- maintained clean working space

- blah blah blah

(Except the job descriptions would be much more eloquent.)
Your name.

address.

phone number.

your career goal.

references.

previous address.

schools you went to.

and then sign it!!
You definitely need to be more specific. If you are trying to write a resume, there is a template in Word that can be used as a guide. Also you can do a search for sample resumes online.

If you are talking about the hierachy of the programs there is no particular order in which to list them.
